ABSTRACT

RESUMO Em janeiro de 2020, a Organização Mundial da Saúde declarou a Covid-19 como emergência de saúde pública no mundo. Diante da ausência de medidas farmacológicas, a única prevenção adotada foi o distanciamento físico. Porém, trabalhadores essenciais ficaram submetidos às políticas de gestão de saúde e segurança das empresas. O objetivo deste artigo é investigar as condições de saúde e segurança dos trabalhadores expostos ao Sars-CoV-2, por meio da aplicação de ferramentas de inovação tecnológica, para dar suporte e subsidiar ações de mitigação de risco da doença. Trata-se de estudo transversal, conduzido na plataforma REDCap, por instrumento autoaplicável de comunicação de risco de trabalhadores em atividade presencial e remota, no Brasil. Participaram 2.476 trabalhadores, dos quais, 723 foram aceitos por análise de consistência das respostas. A idade média foi de 43,5 anos, sexo feminino (53,3%), cor branca (62%), carga de 21-40 horas semanais (60%) e Covid-19 em 27,4% da amostra. A maioria (75,2%) considerou que a transmissão ocorreu no trabalho e que medidas de proteção coletiva foram insuficientes. Os achados apontam deficiências nos planos de contingência das empresas, que repercutem em insegurança e risco de exposição ao Sars-CoV-2, reduzindo a eficácia das medidas sanitárias e transformando o trabalho em lócus de disseminação do vírus.


ABSTRACT In January 2020, the World Health Organization declared COVID-19 a public health emergency worldwide. Given the absence of pharmacological measures, the only prevention adopted was physical distancing. However, essential workers were subject to the health and safety management policies of companies. The aim of this article is to investigate the health and safety conditions of workers exposed to SARS-CoV-2, using the application of technological innovation tools, to support and subsidize disease risk mitigation actions. This is a cross-sectional study, conducted in the Redcap platform, by a self-administered instrument of risk communication of workers in face-to-face and remote activities, in Brazil. A total of2,476 workers took part, 723 of whom were accepted by analysis of consistency of responses. The average age was 43.5 years, female (53.3%), white (62%), working 21-40 hours a week (60%) and Covid-19 in 27.4% of the sample. Most (75.2%) considered that the transmission occurred at work and the collective protection measures were insufficient. The findings point to deficiencies in the contingency plans of the companies, which have repercussions on the insecurity and risk of exposure to SARS-CoV-2, reducing the effectiveness of sanitary measures and transforming the work into locus of dissemination of the virus.

ABSTRACT

BACKGROUND: In this study, the annual leukemia mortality rate is estimated by occupational groups potentially exposed to benzene in Brazil and compared to non-exposed workers by sex. METHODS: Data were extracted from the Mortality Information System and the National Institute of Geography and Statistics from 2006 to 2011. Occupational groups exposed to benzene were defined by using the Finnish Job-Exposure Matrix, FINJEM. RESULTS: We found 21,049 leukemia deaths in 1917 in occupational groups potentially exposed to benzene, corresponding to an annual average mortality rate of 4.5/100,000, higher than the estimate for non-exposed workers: 2.6/100,000, corresponding to a Mortality rate ratio MRR = 1.7. Each benzene-exposed occupational group had increased leukemia mortality, and printers and occupations in graphics presented the highest MRR (2.7), followed by laboratory assistants (MRR = 2.6), laundry workers, chemists, and upholsterers, each of these occupational groups presenting MRR = 2.3. CONCLUSIONS: Benzene shows the need for better enforcement of protective norms against this known carcinogen. Our results support the need for better enforcement of protective norms to reduce benzene exposure.

ABSTRACT

Environmental and occupational exposure to benzene from fuels is a major cause for concern for national and international authorities, as benzene is a known carcinogen in humans and there is no safe limit for exposure to carcinogens. The objective of this study was to evaluate the genotoxic effects of chronic occupational exposure to benzene among two groups of workers: filling station workers (Group I) and security guards working at vehicles entrances (Group II), both on the same busy highway in Rio de Janeiro, Brazil. Sociodemographic data on the workers were evaluated; the concentration of benzene/toluene (B/T) in atmospheric air and individual trans,trans-muconic acid (ttMA) and S-phenylmercapturic acid (S-PMA) were measured; oxidative stress was analyzed by catalase (CAT), glutathione S-transferase (GST), superoxide dismutase (SOD), thiol groups (THIOL) and malondialdehyde (MDA); genotoxicity was measured by metaphases with chromosomal abnormalities (MCA) and nuclear abnormalities, comet assay using the enzyme formamidopyrimidine DNA glycosylase (C-FPG), and methylation of repetitive element LINE-1, CDKN2B and KLF6 genes. Eighty-six workers participated: 51 from Group I and 35 from Group II. The B/T ratio was similar for both groups, but Group I had greater oscillation of benzene concentrations because of their work activities. No differences in ttMA and S-PMA, and no clinical changes were found between both groups, but linearity was observed between leukocyte count and ttMA; and 15% of workers had leukocyte counts less than 4.5 × 109 cells L-1, demanding close worker's attention. No differences were observed between the two groups for THIOL, MDA, MCA, or nuclear abnormalities. A multiple linear relationship was obtained for the biomarkers MCA and C-FPG. A significant correlation was found between length of time in current job and the biomarkers C-FPG, MCA, GST, and MDA. Although both populations had chronic exposure to benzene, the filling station workers were exposed to higher concentrations of benzene during their work activities, indicating an increased risk of DNA damage.

Resumo Introdução: trabalhadores estão sujeitos a riscos decorrentes da exposição ambiental e ocupacional ao benzeno, situação ainda mais crítica quando associada a fatores como desigualdade social, múltipla exposição, suscetibilidade individual, degradação ambiental e às possíveis interações entre os contaminantes ambientais. Além disso, nem sempre as regulamentações são estabelecidas e/ou atualizadas apenas a partir de achados científicos, pois são processos permeados por conflitos de interesses. Objetivo: realizar análise crítica sobre a identificação de risco e avaliação da exposição ao benzeno. Métodos: revisão bibliográfica e documental do marco normativo brasileiro e internacional sobre exposição ao benzeno, com relação aos aspectos toxicológicos, de exposição ocupacional e ambiental e de avaliação de risco. Discussão: o controle da exposição ao benzeno permeia o campo da disputa técnico-política de saúde e segurança, constituindo-se em marco regulatório resultante de consenso de entendimentos. Esses entendimentos são marcados pela disputa entre a proteção à saúde e a gestão empresarial das condições de trabalho, em detrimento das evidencias científicas. Em tal contexto, os Limites de Exposição Ocupacional podem variar significativamente entre os países e agências, apesar do reconhecimento universal de que não há limites seguros para exposição ao benzeno, por ser um composto carcinogênico.


Abstract Introduction: workers are subject to risks from environmental and occupational exposure to benzene. This situation is even more critical when combined with social inequality, multiple exposure, individual susceptibility, environmental degradation, and possible interactions between environmental contaminants. In addition, regulations are not always established and/or updated only from scientific evidences, because they are processes permeated by conflicts of interest. Objective: to produce a critical analysis on risk identification and benzene exposure evaluation. Methods: literature and documentary review of the Brazilian and international regulatory standards on benzene exposure, toxicological aspects, occupational and environmental exposure, and risk evaluation. Discussion: benzene exposure control permeates the technical-political dispute of health and safety, resulting in a regulatory standard from consensus of understandings. These understandings are shaped by the dispute between health protection and business management of working conditions, to the detriment of scientific evidence. In this context, the Occupational Exposure Limits can vary significantly between countries and agencies, despite the universal recognition that there are no safe limits for benzene exposure, because it is a carcinogenic compound.

Resumo Introdução: O benzeno é uma importante fonte de contaminação ambiental e está presente na composição da gasolina, expondo trabalhadores de Postos de Revenda de Combustíveis (PRC) a intoxicações. Objetivo: relatar experiências de vigilância da exposição ao benzeno do Centro de Referência em Saúde do Trabalhador da região dos Vales/RS. Métodos: a vigilância no território incluiu o mapeamento regional dos PRC, ações intrasetoriais, intersetoriais e interinstitucionais, inspeção nos locais de trabalho, desenvolvimento de instrumento de intervenção específico para esse ramo produtivo - Notificação Recomendatória, avaliações individuais dos trabalhadores - e outras abordagens visando intervir nos ambientes com exposição ao benzeno. Resultados: foram observadas alterações em biomarcadores de efeito e de exposição, além de inadequações dos PRC no monitoramento clínico e laboratorial dos trabalhadores. Nos ambientes e processos de trabalho ficaram evidentes situações que ampliam a exposição: inadequações nas pistas de abastecimento, uso de "flanela", inexistência ou insuficiência de medidas de proteção coletivas, ausência de medição eletrônica dos tanques e de higienização dos uniformes por parte das empresas, dentre outras. Conclusão: as experiências desencadeadas por meio de ações integradas no setor saúde demonstraram a potencialidade benéfica da vigilância da exposição ao benzeno em PRC.


Abstract Introduction: Benzene is an important source of environmental and occupational contamination and may cause poisoning to gas station workers. Objective: to report the benzene exposure surveillance experience carried out by the Centro de Referência em Saúde do Trabalhador (Occupational Health Reference Center) of Região dos Vales, in Rio Grande do Sul, Brazil. Methods: surveillance activities included regional mapping of gas stations, intrasectoral, intersectoral and inter-institutional actions, workplace inspections, development of a specific interventional instrument for this productive branch, and other approaches aiming to intervene in benzene exposure environments. Results: we observed alterations in biomarkers of effect and of exposure, besides companies inadequacies in clinical and laboratory monitoring of workers. In the work processes and the work environments inspected, situations that enlarged exposure were evident: inadequacies in fuel supply lanes, use of cloth strip for fuel leakage absorbance during tank filling, lack or shortage of collective protection measures, absence of volume electronic measurement of the storage tanks and absence of work cloths washing by the companies, among others. Conclusion: experiences unleashed through integrated actions in the health sector demonstrated the beneficial potentiality of the benzene exposure surveillance in gas stations.

Resumo Introdução: o Roteiro de inspeção sanitária de ambientes e processos de trabalho em postos de revenda de combustíveis (RIAT-PRC) é utilizado para fins de vigilância em saúde pelo Centro de Referência em Saúde do Trabalhador (Cerest). Objetivo: descrever os usos e aplicações do RIAT-PRC a partir da experiência da vigilância em saúde do trabalhador do estado de Santa Catarina. Método: relato descritivo e analítico do instrumento RIAT-PRC e da avaliação de sua qualidade e uso no estado de Santa Catarina, no período de 2010 a 2014. Resultados: o roteiro foi aplicado em 459 PRC, com bom grau de preenchimento no geral. A aplicação do instrumento permitiu identificar baixa realização de monitoramentos biológicos e ambientais por parte das empresas. Também permitiu o acompanhamento das condições de trabalho e de saúde dos trabalhadores ao longo do tempo, bem como a possibilidade de identificar fatores e situações de risco nos ambientes de trabalho. Conclusão: o RIAT-PRC mostrou factibilidade para vigilância e para subsídio de estudos sobre exposição ao benzeno em PRC. Evidenciou-se a necessidade de aprimorar a capacitação das equipes técnicas para a efetiva vigilância da exposição ao benzeno e a outros agentes químicos presentes nos PRC.


Abstract Introduction: the Form for Sanitary Inspection of Work Processes and Environments in Gas Stations (RIAT-PRC) is used for health surveillance by Reference Centers in Occupational Health (Cerest). Objective: to describe the uses and applications of the RIAT-PRC tool from the experience of the Surveillance of Occupational Health in the state of Santa Catarina, Brazil. Method: descriptive and analytical report of the RIAT-PRC tool and of the evaluation of its quality and use in Santa Catarina, from 2010 to 2014. Results: the form was introduced in 459 gas stations, with good filling degree in general. The application of the tool, made it possible to verify that the companies carried out only a few biological and environmental monitoring. It also allowed us to follow workers' occupational and health conditions along the study time as well as to identify risk factors in work environments. Conclusion: the RIAT-PRC was useful for surveillance and for supporting studies on benzene exposure in gas stations. The results highlighted the need to improve the training of surveillance technical teams, aiming at an effective monitoring of gas stations workers' exposure to benzene and other chemicals.

Resumo Introdução: trabalhadores de postos de revenda de combustíveis estão expostos a compostos orgânicos voláteis, dos quais se destaca o benzeno. O manejo clínico do amplo espectro de sinais e sintomas desses trabalhadores ainda é um desafio para assistência em saúde. Objetivo: apresentar proposta de classificação clínico-laboratorial baseada na análise de hemograma e na avaliação clínica dos trabalhadores, para orientar a conduta dos profissionais de saúde. Métodos: a classificação clínico-laboratorial foi definida a partir de critérios de hierarquização dos resultados das avaliações clínicas e de vigilância à saúde de modo a possibilitar a organização dos procedimentos e fluxos de atenção à saúde. Resultados: a proposta define o manejo clínico com base na classificação dos expostos ao benzeno em quatro níveis de alterações clínico-laboratoriais que determinam os procedimentos compatíveis com o grau da exposição e de seus efeitos à saúde: 1) sem alterações significativas; 2) alterações inespecíficas ou incompletas, 3) alterações clínicas e/ou laboratoriais; 4) alterações sugestivas de benzenismo. Conclusão: a proposta de classificação dos achados clínico-laboratoriais é potencialmente capaz de orientar a conduta médica na avaliação das condições de saúde e dos riscos da exposição ao benzeno e de subsidiar o monitoramento sistemático e contínuo necessário para a definição de rotinas assistenciais para os coletivos de trabalhadores.


Abstract Introduction: gas stations workers are exposed to volatile organic compounds, especially benzene. Clinical management of the broad spectrum of symptoms and signs observed in these workers is still a challenge for health care. Objective: to present a clinical-laboratory classification, based on hemogram analysis and clinical evaluation of workers exposed to benzene, as guidelines for health care professionals. Methods: the clinical-laboratory classification was defined based on priority-setting criteria for the results of clinical evaluations and health surveillance in order to allow the organization of health care flows and procedures. Results: the proposal defines the clinical management based on classification of workers exposed to benzene in four levels of clinical and laboratory alterations that determine procedures according to level of exposure and health effects: 1) without significant alterations; 2) unspecific or incomplete alterations; 3) clinical and/or laboratory alterations; and 4) alterations suggesting benzene poisoning. Conclusion: the proposal for classification of clinical and laboratory findings is potentially able to guide medical practice in the assessment of health conditions and risks of benzene exposure, and subsidize the systematic and continuous monitoring necessary for establishing health care practices offered to workers.

ABSTRACT

O objetivo deste artigo é apresentar a experiência da rede de vigilância da exposição ao benzeno em Postos de Revenda de Combustíveis, descrevendo seus componentes, desenvolvida no Sistema Único de Saúde. Relato e análise de experiência de vigilância e seus impactos à saúde dos trabalhadores de postos de combustíveis em seis estados do Brasil. Apresenta a perspectiva de ação operada pela circulação de informações, encontros nacionais, discussões de estratégias particulares e compartilhadas das experiências, metodologias e instrumentos comuns. São descritos procedimentos de inspeção, avaliações individuais e formação. Foram identificadas situações de risco e exposições ocupacionais e ambientais em 1.312 postos e 564 trabalhadores foram avaliados. Estavam expostos frentistas, chefes de pista e encarregados de medição, descarga e testes. As características integradas e complementares desta vigilância, articuladas às áreas ocupacional e ambiental, contemplam a intervenção a territórios ampliados e conectados em rede. A intervenção supera as fragmentações entre práticas individuais e coletivas e entre serviços e ensino, tornando-se um exemplo de ação para avaliação e redução do impacto à saúde em trabalhadores, pela articulação da ação sanitária, ambiental e de conexão intersetorial.

The intervention would transcend fragmentation between individual and collective practices, services and education, becoming an example of action for assessment and mitigation of the impact on health workers, the articulation of action in health, the environment and intersectorial connection.
